| Description | Description of the disability. |
| Medical Code | Code of the related medical condition, such as Blind, Deaf, Prosthetic Limb.
List Medical Code Validation (STVMEDI) |
| Service Code | Code of the related disability service need, such as County Paratransit Service, Hearing Aid Required, Disabled Student Services.
List Disability Service Validation (STVSPSR) |
| Primary | Check box used to indicate whether this is the primary disability for the student. |
| Activity Date | Date on which the record was last updated. Display only. |
